Provides individualized, continuous, long-term treatment, rehabilitation and support services to individuals with severe and persistent mental illness Collaborates with the Forensic Outreach Program to serve individuals bound by an order of the Ontario Review Board Assists individuals to make improvements in functioning, to better manage symptoms and to set and achieve personal goals
