Offers specialized assessment and support for individuals experiencing mental health difficulties Treatment plans may include: individual psychotherapy addictions counselling groups (educational and/or therapeutic) case Management counselling, education and support for individual family members specialized geriatric services (assessment and referral) seniors intensive case management Behavioral Support Ontario (BSO) Seniors Crisis service psychiatric resources court support services Rapid Access Addiction Medicine Clinic (RAAM) Integrated Crisis Response Team Crisis Support Workers work with individuals to determine an immediate and short-term plan to help stabilize the crisis, and work with other community resources to prevent the re-occurrence of crisis and develop capacity for service pathways to meet individuals" needs.
