Services for persons requiring long term health care support to promote independence and quality of life including:24 Hour Assisted Living: for frail, high-risk seniors requiring services to prevent long hospital stays and reduce emergency room visits or permanent placement in Long Term Care. Seniors can receive personal care, homemaking, meals, assistance with medication and safety checks as needed.24-Hour Attendant Services: to adults with physical disabilities including bathing and washing, personal grooming, mobility/transferring, meal preparation, laundry assistance, light housekeeping and grocery shopping.Outreach Program: personal support and homemaking services for physically disabled people living in the community. Can be provided in an individual"s home, work place, educational institution or at The Friends Respite Unit. Clients must be 18 years or older so they can take legal responsibility for their own care.Respite Apartment Program: Two-bedroom fully accessible unit providing support for people requiring short term respite. Respite Program: provides caregiver relief - services include supervision, meals and activities either in the persons home or at the Forest Hill Apartments. Service available on a daily, 24 hour basis.Seniors" Homemaking Program: provides light housekeeping, meal preparations, security checks. Some personal care for seniors who are able to direct own care and wish to remain in their own home.Post Stoke Program: helps individuals who have had stroke get back to daily activities with support of a Community Navigator.Adult Day Away Program: provides social and recreational activities in group settings for seniors and adults with physical disabilities and those suffering from age related dementia including Alzheimer"s. Program offer respite to caregivers. Rides to the programs may be able to be arranged. Call for locations and times. Fee of $15 - virtual programming is free. Groups run Thursdays from 9 am-3 pmCaregiver Support Program: provides education, peer support, group and one-on-one support for family caregivers.Accessible Housing: in Parry Sound and Muskoka providing 24-hour support, call for application.P.A.T.H. (Priority Assistance to Transition Home) provides transportation and trained PSW attendant to assist in transitioning home from hospital
