Primary Care Outreach - Physicians and Nurse Practitioners assess and treat non-life-threatening injuries or illnesses Geriatric Assessment Intervention Network (GAIN) - A network of coordinated healthcare services and partners working together to strengthen the supports and capacity of frail older adults. Memory Clinic (MINT) - team approach for the early diagnosis, treatment and support of problems associated with memory loss. Diabetes Education Programs - One-on-one education and supports, and groups for people living with diabetes Counselling Services - Mental Health and Addiction counsellors offer individual therapy and group sessions Dental Services - subsidized basic dental services for uninsured persons - incudes teeth cleaning, check-ups, tooth removal and fillingsCommunity Programs Include: Nutrition and Healthy Eating - individual counselling Food Security program Cooking classes Arthritis Clinic Caregiver Support Group Women"s Book Club Mindfulness Groups Exercise Programs Smoking Cessation Treatment
