We provide Primary Health Care, system navigation and support for diverse populations of all ages and stages of life with a focus on those who experience challenges and barriers to accessing care, including those living in poverty; experiencing mental health and addictions; housing instability; isolated seniors; youth at risk and those with complex chronic disease.Mobile Health Outreach Bus (MHOB):Team consists of an outreach worker, primary care providers andcommunity partners. who work collaboratively to bring services toclients in the community. Services provided include wound care, support with form completion, community resources/referrals, mental health support, harm reduction and personal hygiene supplies.
