Assists individuals with a primary diagnosis of severe and persistent mental illness to live independently in the communityCase Management Works with individuals to improve their quality of life and level of wellness Provides services in a place of individual"s choice, mainly in the community Focuses on individuals with mental illness who are homeless or at risk of becoming homelessProvides assistance in the following areas: Assistance with securing affordable housing Consultation and assessment Coordination and advocacy with other supports and services Crisis support Financial assistance and budgeting Family support Interpersonal support Improving living skills Medication monitoring Supportive counsellingCommunity Treatment Orders (CTO) Provides a treatment and service plan issued by a physician in consultation and coordination with the individual and service providers Provides case management services and supportSupportive Housing Assists individuals in acquiring and maintaining housing Offers flexible and individualized supports that vary in intensity Collaborates with other service providers to ensure individuals have access to safe affordable housingPeer Support Coordinates group sessions for individuals with mental illness and/or addictions issues to share experiences Offers meetings with the peer support worker in an individual or group setting
