Program
Provides services for individuals with Alzheimer"s disease and other related dementias, and their families. First Link - a partnership program to provide direct referrals that link individuals diagnosed with Alzheimer"s disease or related dementias to a community of learning, services, and support as early as possible in the disease process Behavourial Supports Ontario - provides enhanced and integrated services meeting the needs of older adults with responsive behaviours Counselling - for those who have questions about dementia, are concerned they may have dementia, have been diagnosed, or are caring for someone with dementia Cognitive Assessment - can be administered in the office, with or without a referral from your primary care provider Support Groups - to share experiences and learn new coping strategies Day Program - provides a safe and stimulating environment for those affected by Alzheimer"s disease and related dementias In-Home Respite Care - a non-emergency support program that is tailored to meet individual needs in their own home Education Programs - to assist with the distribution of information to people living with dementia, formal and informal care partners, students and the general public Lending Library - available for those who are interested in learning more about Alzheimer"s disease or related dementias through books, videos and resource guides Finding Your Way - will provide a safety kit which contains tools to help ensure the safety of the individual, while allowing them to maintain their independence and dignity MedicAlert - a national program designed to help identify those who are lost and assist in a safe return home In-Home Mobility Monitor - offers a safe alternative to monitoring the person in your care when in your home Chatham Day Program36 Memory LaneMon-Sat 9:30 am-3 pmWallaceburg Day Program212 Elizabeth StTue-Fri 9:30 am-3 pm
