Primary Care Medical clinic provides comprehensive team based health care services primary care is offered through Nurse Practitioners in collaboration with an inter-disciplinary health care team of registered nurses, community mental health workers, dietitians, physiotherapists, chiroprators, and a collaborating physician detailed health assessment to make a diagnosis treatment plans and medication recommendations/prescriptions blood work collected on site immunizations chronic disease management, support and prevention counselling well baby and well-mom visits referrals to medical specialists nutritional counselling mental health services and supports spirometry services smoke cessation supports and counselling specialized hypertension services Intensive care coordination and support with system navigation is also offeredPartnerships include: CardioPREVENT program in partnership with the Ottawa University Heart Institute Low Back Pain Chiropractic services as part of the Provincial Low Back Pain Program Complex diabetes clinic in partnership with the Cornwall Community Hospital Access to psychiatric services and consultation in conjunction with the Royal Ottawa Hospital
