Team-based family and community primary care. Clinical team includes family physician, nurse practitioner, nurse, dietitian, social worker, pharmacist, physio and occupational therapist, chiropodist/foot care nurse. Programs for Family Health Team patients: mental health counselling for those without private coverage footcare for those without private coverage physiotherapy for those not qualifying for OHIP chronic pain management COPD Healthy Heart nutrition counselling smoking cessation (eligibility for free nicotine replacement therapy) medication management/counselling vaccination follow-up after hospitalization Other programs, workshops and information sessions are available to the public.
