The Transitions in Care Program is a voluntary program that provides telephone follow-up with youth and / or their guardians after discharging to community from an inpatient program within our portfolio (Step Up Step Down, Unit 39, and PChAD)., , During these phone calls assessment screening tool is used evaluate present moment risk, a conversation occurs to see how the youth is doing since their discharge, and to help aid in any resources / referrals that are needed.
