Three community health centers located across Scarborough * Staffed by doctors, nurse practitioners, registered nurses, social workers, dietitians, chiropodists (foot care specialists), and medical secretaries, they provide medical care and health-related services such as check-ups, immunizations, diabetic foot care, nutrition counselling, social support services, and harm reduction (free condom program, needle exchange, point of care HIV testing) * services provided free of cost to clients with OHIP, Interim Federal Health (IFH), and clients with no status * In-person and virtual services are offeredPrimary Health Care Team services include:preventative health care - screening, smoking cessation, teaching and educationroutine physical exams and PAPsimmunization for children and adultsassessment and treatment of acute and episodic illnesseson-going management of chronic diseases and the development of self-management skillspre and post-natal care, and well-baby caretriage service for urgent client medical conditionschiropody servicestherapeutic counselling one-on-one or with familynutrition assessment and counsellinghealth education - healthy lifestyle and diabetes managementafter-hours phone consultationinformation, advocacy, service referral and practical supportflu immunization clinics for CHC clients and surrounding community
