Community Mental Health Centers
Provides housing and support to adults with serious mental illnesses and addictions. Services include apartment and outreach programs, rent-geared to income residential programs and various group service programs with a focus on decreasing social isolation and promoting strengths and skill development.Apartment Program - rent subsidies and outreach support for people living with a serious mental illness, who are homeless or at risk of homelessness and who need and can benefit from housing and outreach support services provides subsidized rent for nearly 100 apartments in Owen Sound and other areas throughout Grey-Bruce (the number of available apartment units in each community varies over time so all inquiries for program are best directed to the central office) leases the units from private sector landlords and then sublets them to program recipients rent for the apartments does not exceed the Ontario Disability Support Program or Ontario Works maximum shelter allowance (unless an individual is working and then the apartment is based on their income) water, hydro and heat are included in the rental amount provides recipients with one-to-one outreach support in addition to the subsidized rental apartments program staff provide supportive counselling, mental health assessment, practical assistance with activities of daily living and skills teaching to assist clients in their recovery and in developing further skills as determined by the clientOutreach Support - one-to-one support for adults living with serious mental illness, including: supportive counselling practical assistance with activities of daily living and skills teaching for independent living assistance in finding and keeping suitable, affordable housing social and health related advocacy and social connection support is variable in frequency and geared to client need. Clients reside in various settings in the community, including individual apartments services are delivered by Outreach Workers across Grey and Bruce CountiesResidential Programs - two sites provide independent skill teaching and goal setting for adults living with a serious mental illness staff provide practical assistance, one-to-one support and skills teaching to assist residents in their recovery, in developing skills for independent living and improving self sufficiency clients learn to manage their medication, increase their independently, to live cooperatively and to manage household responsibilities Brooke House in Owen Sound provides affordable housing and staff support 7 days a week (but not overnight) for eight adults. All residents have individual bedrooms and shared common areas Frank Street home in Wiarton provides affordable housing and staff support 7 days per week (but not overnight) for 10 adults. All residents have individual bedrooms and shared common areas Community Homes for Opportunity - there are three homes that are located in Meaford, Hanover and Owen Sound, providing skill teaching and development, goal setting and outreach services for adults living with a serious mental illness and/or addiction CMHA GB staff, working in partnership with the homeowners and their staff, provide practical assistance, one-to-one support and skills teaching to assist residents in their recovery, in developing skills for independent living and improving self sufficiency staff are on site 7 days a week
