The team consists of Doctors; Nurse Practitioners; Registered Nurses; Occupational Therapists; a Health Promoter; Registered Dietitians; Mental Health Counsellors.Area residents have access to GBFHT"s services by getting a referral from their GBFHT Family Physician (only patients of affiliated physicians may be referred). Family physician referral also required for most clinics. GBFHT is the regional Diabetes Education Centre (DEC). All patients with diabetes regardless of who their family doctor is may be seen at the DEC.Services include:Counselling: Individual and marital counselling available, including child and youth mental health counsellors for short-term mental healthcare. Patients 16 years and under can be helped with anxiety, depression, anger, acting out, conflicts with parents/siblings, addictions, as well as problems with school and socialization. Offers one-on-one or family sessions. Single session counselling available without appointment on first come, first served basis.Clinics: Adult Counselling Child/Youth Counselling Diabetes Care and education Nutritional Counselling Chronic Disease Self Management Program Smoking Cessation Program Hypertension Clinic Heart Failure Clinic Cardiac Care Clinic Wound Care Clinic Ontario Telemedicine Network participant Teen Sexual Health Clinic Well Woman Clinic Falls Prevention:The family health team assists seniors with preventing falls or recovering from falls by providing: fall risk assessment/screening at annual physicals including basic falls prevention advice after a fall, identifying the cause of the fall and helping patients develop strategies to prevent future falls referral to other falls prevention programs or services regular medication reviews basic vision assessments and reminders at annual physicals to have regular biannual eye examsThe Georgian Bay Family Health Team is a member of the Staying Independent Falls Prevention Coalition (SIFPC)
