Program designed to help reduce cardiovascular risk factors a mentor will provide personalized, confidential and free intervention plan to achieve a better heart healthy lifestyleA full evaluation of risk factors is done at the beginning and at the end of the three month program during which a mentor will call or email, at a chosen frequency, to monitor and provide information on exercise, nutrition, smoking cessation (if needed) and stress management a one-year follow-up, including updating the cardiovascular risk profile, is offered to all participants a copy of the cardiovascular risk profile is also provided to the referring physician at each time point
